How do I join Four Oaks Pre-School Playgroup?

Please contact playgroup supervisor Heather Moss on 07966 147 584 to arrange a visit to playgroup.  If you like what you see, please complete an Admissions Request form which can be found in playgroup.  You will then be contacted by our Admissions Coordinator and, depending on the start date that you have requested, either allocated a place if one is available or added to our waiting list.

 When you are offered a place, you will be given a pack which contains the following documents for completion and return;

• Confidentiality agreement;

• CRB check form;

• Duty Rota preferences form;

• Child information and emergency contacts form;

• Grant form (if applicable, if not this will be given to you to sign in the term after your child's 3^rd birthday)
